New Registrations

New Registration Information


We welcome registrations from new patients living within our catchment area (please see map and summary). We require you to fill in a GMS1 form and a new patient questionnaire and then bring these into the surgery with the following information: your current address and postcode, previous address and postcode or date of entry into the UK if you are an immigrant, NHS number and 2 forms of ID, one being photographic and another form of ID which contains your current address, this would normally be a household bill.

Please note you will need to bring these forms back in person to reception. We do not accept them by e-mail or post as we need to see your ID.

Children registering at the surgery that are under the age of 16 follow the same process as an adult but will need to have a parent or guardian present during the registration process. If the child you wish to register is aged 6 or under please see our Child Registrations page for more information.

You may also wish to register for our online service through which you can book appointments and order repeat medication. Please complete the SystmOnline registration form and bring it along to the surgery with your other completed registration forms and ID if you wish to use this service. The online service is not available to patients under the age of 16.
